What is being studied
Low back pain is very common and a leading cause of disability. In Australia, it affects nearly four million people, and for about one in three, the pain lasts longer than three months. Current treatments often provide only small and short-term relief.
Why is this research important?
The results will help us understand whether this treatment can be used more widely to improve care for people with ongoing low back pain.
About the trial
New research into how long-term pain develops has led to new treatment approaches. One of these, called graded sensorimotor retraining, has shown promising results in improving pain and daily function in an earlier study. We now want to see how well it works in everyday healthcare settings.
This study will compare this new approach with standard physiotherapy care for people with chronic low back pain.
Participation
Screening Steps
You may be able to take part if you:
- Have low back pain (with or without pain in your leg) as your main problem
- Have had this episode of back pain for at least 3 months
- Have at least moderate pain that affects your daily activities
- Can read and understand English well enough to complete questionnaires
- Have access to the internet and a device such as a smartphone, tablet, or computer
You will not be able to take part if you:
- Have a known or suspected serious condition affecting your spine or nerves
- Are currently (or think you may be) pregnant, or have had a baby in the past 6 months
- Have had back (spinal) surgery in the past 12 months
- Are planning to have major surgery in the next 12 months
- Are unable or unwilling to travel to the study clinics
- Have a medical condition that makes it unsafe for you to take part in physical activity

What does participation involve (post-screening):
- Eligible participants who agree to take part will be randomly allocated to one of three groups: (Group A; graded sensorimotor retraining, Group B; supervised exercise, Group C: Guideline based care)
- Group A: 6 to 10 sessions over 12 weeks, Group B: up to 8 sessions over 12 weeks, Group C: 1 to 2 sessions over 12 weeks.
- Number of sessions
- Assessments at the start, 12 weeks, 26 weeks, 52 weeks.

Age/eligibility
People over 18 with chronic low back pain may be eligible to participate .
